This EP was issued in Britain in 1966 and contained two of the seven sides that she had released on 45 by Blue Rock, a subsidiary of Mercury records USA. The above would have originally come with a company 'picture' sleeve, believe it or not, many of those sleeves were seen as ephemera and discarded by record buyers in the mid to late 20th century.
